We Compared The Features of 137 AI Healthcare Apps: Here's What We Found

EHR integration is nearly universal in AI healthcare apps, but it is not freely accessible in practice. We built a dataset of 137 AI healthcare apps, inspected their public feature information ourselves, classified every feature with a seven-label availability scheme, and ran the aggregates to see what actually matters if you are shipping your own AI healthcare app.

The dataset spans seven workflow families: administrative revenue automation, clinical documentation automation, clinician decision support, imaging and pathology diagnostics, patient engagement and coaching, patient symptom triage, and specialty monitoring and diagnostics. For each tool, we captured a comparable feature taxonomy across documentation, integration, diagnostics, reasoning, triage, communication, monitoring, and behavioral support so the analysis reflects packaging rather than marketing language.

Summary

This study analyzes the feature landscape of 137 AI healthcare apps captured from their public feature information. We included tools across administrative revenue automation, clinical documentation automation, clinician decision support, imaging and pathology diagnostics, patient engagement and coaching, patient symptom triage, and specialty monitoring and diagnostics, then classified 12 feature categories with a standardized availability scheme.

EHR and practice management integration is the closest thing to a universal feature in AI healthcare apps at 123 of 137 tools, which means integration is now a baseline buyer expectation rather than a differentiator.

That baseline is still heavily controlled: 94 of the 123 EHR implementations are restricted, which confirms that integration access is usually tied to deployment, enterprise contracts, care settings, or partner relationships rather than open self-serve use.

Clinical documentation automation is the most internally complete workflow. Structured clinical note generation appears in 24 of 24 tools, ambient capture appears in 23 of 24, and EHR integration appears in 24 of 24, which means the core scribe workflow has already commoditized inside that sub-category.

Clinical reasoning is the broadest intelligence layer after EHR integration at 64 of 137 tools, which suggests that evidence retrieval and decision support are spreading across workflows faster than narrow operational features.

Diagnostic image analysis is a highly monetized feature: 30 of the 33 present implementations are paid only, which makes imaging triage one of the clearest premium capabilities in AI healthcare apps.

Digital pathology is both rare and fully paywalled. It appears in only 7 of 137 apps, all paid only, which makes it the strongest rare-and-premium signal in the dataset.

Prior authorization is surprisingly underbuilt for its operational importance. Only 9 of 137 apps show it, and 5 of those are in administrative revenue automation, which suggests the feature remains concentrated in back-office workflows rather than spreading into broader clinical platforms.

Symptom assessment has the strongest consumer-facing free signal. It appears in 27 apps, and 13 of those implementations are either free full or free limited, which confirms that patient triage is the clearest place where free access functions as product distribution.

Behavioral health coaching is concentrated almost entirely in patient engagement and coaching. Nineteen of the 21 apps that offer it sit in that workflow, which means behavioral support is not yet a horizontal layer across AI healthcare apps.

Remote monitoring is more common than diagnostic image analysis, at 51 of 137 apps versus 33 of 137, which means longitudinal risk tracking has become a bigger horizontal surface than many diagnostic point solutions.

Paid-only availability dominates provider, enterprise, and regulated clinical workflows, which means builders should treat free access as a patient-facing or clinician-reference mechanic, not the default commercial model for AI healthcare apps.

We built this dataset from scratch. For each of the 137 AI healthcare apps, we inspected public product information and recorded the primary workflow, business model, and availability of 12 feature categories: ambient visit capture, structured clinical note generation, EHR and practice management integration, medical coding and revenue-cycle automation, prior authorization, diagnostic image analysis, digital pathology, clinical reasoning, symptom assessment, patient communication, remote monitoring, and behavioral health coaching. Each feature was classified with one standardized availability label, and the full comparison table is below.

We analyzed 137 AI healthcare tools based on publicly available information from their homepages, product pages, feature pages, documentation pages, app listings, and pricing pages.

We include tools whose primary value proposition is to use AI for healthcare-related workflows, including symptom guidance, clinical documentation, patient intake, care navigation, medical triage, health coaching, diagnostics support, treatment planning, practice operations, or patient communication. We exclude generic medical apps, wellness apps, fitness apps, telehealth platforms, EHR systems, healthcare practice software, and administrative tools unless AI-powered healthcare support is a central advertised feature.

The dataset includes tools used by clinicians, care teams, health systems, payers, practices, patients, and consumers when healthcare-specific functionality is central to the product. This includes clinical documentation automation, clinician decision support, patient symptom triage, patient engagement and coaching, administrative revenue automation, imaging and pathology diagnostics, and specialty monitoring and diagnostics.

We excluded general-purpose AI assistants, generic transcription tools, generic analytics platforms, generic scheduling tools, wellness apps without a clear healthcare workflow, and broad productivity software unless healthcare-specific use cases were presented as a central advertised feature. For ambiguous tools, we included a tool only when a buyer or user would reasonably describe it as an AI healthcare product rather than as a general AI, productivity, analytics, or consumer wellness product.

The dataset is designed to represent the most visible, relevant, and commercially meaningful products in the AI healthcare software market. A small number of niche, regional, newly launched, stealth, or narrowly deployed products may have been missed, but the sample is structured to capture the main competitive patterns across the category rather than every marginal edge case.

AI healthcare tools often describe similar capabilities with very different terminology. For example, clinical documentation products may use terms such as ambient scribe, AI medical scribe, note automation, visit summarization, charting assistant, or clinical documentation assistant. To make the analysis comparable, we grouped vendor-specific wording into broader feature categories that reflect the underlying user workflow.

This categorization avoids two common problems: treating every vendor-specific phrase as a separate feature, which would make the analysis too fragmented, and using overly broad categories, which would obscure meaningful differences between clinical, administrative, diagnostic, monitoring, and patient-facing products.

For each tool, we captured 12 feature categories: ambient visit capture and transcription, structured clinical note generation, EHR and practice management integration, medical coding and revenue-cycle automation, prior authorization and payer documentation, diagnostic image analysis and triage, digital pathology image interpretation, clinical reasoning and evidence retrieval, symptom assessment and care navigation, patient communication and scheduling automation, remote monitoring and risk prediction, and behavioral health coaching and escalation.

For each feature, we applied a standardized availability label based on the information published by each vendor. Absent means the feature is not available, or does not appear to be available, based on public information. Free full means the feature is available for free without meaningful usage limits. Free limited means the feature is available for free, but with usage, volume, functionality, workflow, or access limits.

Paid only means the feature is available only through a paid plan, enterprise contract, paid product, paid device, paid deployment, or commercial arrangement. Trial only means the feature is available only during a free trial or temporary evaluation period. Restricted means the feature depends on a specific integration, care setting, provider relationship, device, geography, payer relationship, clinical deployment, enterprise contract, beta program, or other access condition. Unclear means the feature appears to be present, but public information does not clearly indicate whether it is free, paid, trial-based, limited, or restricted.

When public information was incomplete or ambiguous, we avoided inferring availability beyond what could reasonably be supported by the vendor's own materials. In those cases, we used the Unclear label rather than assuming that a feature was free, paid, restricted, or fully available.

For the quantitative analysis, we counted a feature as present whenever it was labeled Free full, Free limited, Paid only, Trial only, Restricted, or Unclear. We counted it as not present only when it was labeled Absent. Percentages were calculated both across the full dataset and, where relevant, within each primary workflow category.

For availability mix calculations, we used only the tools where the feature was present as the denominator. This means the percentage breakdown for Free full, Free limited, Paid only, Trial only, Restricted, and Unclear reflects the distribution among products that actually offer the feature, not among all products in the dataset.

Finally, we reviewed the resulting patterns at both the market level and the workflow-category level to identify which features are broadly adopted, which are usually free or limited, which are usually paid or restricted, and which remain rare or differentiated within specific AI healthcare segments.
